Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Minneapolis, MN.

You will plan, organize, develop, and direct OT services to meet federal and state standards and ensure the highest quality resident care. This role requires collaboration with the interdisciplinary team and strict documentation and compliance with regulations.

Qualifications

• Must hold a degree in Occupational Therapy.
• Active OT license in Minnesota (or practicing state).
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Screen residents to determine best OT interventions.
• Evaluate residents and develop treatment plans with goals.
• Communicate evaluation findings and plans to residents, families, and team.
• Create individual therapy schedules with the care team.
• Provide therapy per plan and document and bill services.
• Perform other duties to effectively treat patients.
• Oversee COTA as needed and ensure proper documentation and reimbursement.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development to maintain licensure and certification
