Job summary

Evolve Therapy Services is seeking an Occupational Therapist to lead in-house therapy services at a skilled nursing facility in Minneapolis, MN. The role involves planning, organizing, and directing OT services to maintain high-quality resident care in compliance with standards and regulations.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Evaluate residents to inform treatment plans and set goals.
• Interpret and communicate evaluation findings and treatment plans to residents and families.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to the treatment plan and document/bill services.
• Perform other duties as assigned to treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement practices.
