Job summary

Evolve Therapy Services, LLC is seeking a licensed Occupational Therapist to lead in-house therapy services at a Skilled Nursing Facility in Minneapolis, MN. You will plan, organize, and direct OT services to ensure high-quality resident care in compliance with federal, state, and local standards.

The role requires an active OT license in the practicing state, a degree in OT, and commitment to ongoing professional development.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to develop and adjust treatment plans with goals.
• Communicate evaluation findings, plans, and recommendations to residents, families, and the team.
• Establish individual therapy schedules with other team members.
• Deliver therapy per treatment plan and ensure proper documentation and billing.
• Oversee COTA and ensure documentation, treatments, and reimbursement practices are followed.
• Perform all other duties as assigned to effectively treat patients.
