Job summary

Evolve Therapy Services in Minneapolis, MN is seeking a dedicated Occupational Therapist to lead in-house skilled nursing facility services. You will plan, implement, and supervise OT programs to help residents maintain independence in collaboration with a multidisciplinary care team.

An active Occupational Therapy license in Minnesota is required, with commitment to ongoing professional development and compliant documentation.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in Minnesota is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Continuing education and professional development to maintain licensure and certification
