Job summary

Evolve Therapy Services, LLC in Erie, PA is seeking an Occupational Therapist for an in-house Skilled Nursing Facility on a PRN basis. You will plan, direct, and coordinate OT services to ensure high-quality resident care in accordance with federal and state standards.

You will screen residents, develop and adjust treatment plans, communicate progress to families and the interdisciplinary team, and supervise COTA as needed to maintain compliant documentation and reimbursement practices.

Qualifications

• Must have a Degree in Occupational Therapy.
• Must hold an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy Services in the facility.
• Conduct regular screenings to determine best intervention for residents.
• Evaluate residents to inform treatment plans with short- and long-term goals.
• Communicate findings and plans to residents, families, and the care team.
• Establish resident therapy schedules with other team members.
• Conduct therapy per plan and ensure proper documentation and billing.
• Oversee COTA and ensure proper documentation and reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
