Evolve Therapy Services seeks a licensed Occupational Therapist for an in-house Skilled Nursing Facility serving Grand Island and Central City, NE. Full-time salaried position with flexible hours; part-time or PRN options available.

Responsibilities include evaluating residents, developing and implementing treatment plans, communicating with families and the interdisciplinary team, ensuring documentation and accurate billing, and supervising COTA as needed.

Qualifications

• OT degree required and active license in practicing state.
• Commitment to ongoing professional development to maintain licensure.
• Ability to evaluate residents and develop treatment plans in SNF setting.

Responsabilités

• Screen residents to determine interventions and treatment.
• Evaluate residents and develop short- and long-term goals.
• Communicate findings and plans to residents, families, and team.
• Schedule resident therapy with team coordination.
• Document therapy and ensure proper billing practices.
• Carry out treatment per plan and supervise COTA as needed.
• Oversee documentation and clinical reimbursement processes.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Full Time (Flexible Hours + Salaried!), Part Time, or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
