Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Grand Island/Central City, NE. You will plan, organize, develop, and direct OT services to ensure high-quality resident care aligned with standards.

This role offers Full Time, Part Time, or PRN options with competitive pay. Must hold an active OT license in practicing state(s) and a degree in Occupational Therapy, with ongoing continuing education requirements.

Qualifications

• Must have an Active Occupational Therapy license in practicing state(s)
• Degree in Occupational Therapy required
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team
• Establish individual resident therapy schedules with other team members
• Conduct therapy according to treatment plans and ensure proper documentation and billing
• Perform all other duties as assigned to effectively treat patients
• Oversee COTA and ensure documentation, treatments, and reimbursement practices meet standards

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time (Flexible Hours + Salaried!), Part Time, or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
