Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Omaha, Nebraska. The role involves planning, directing OT services, and ensuring care meets federal, state, and local standards.

You will work with an interdisciplinary team to optimize resident outcomes and ensure proper billing and documentation. Requires a Degree in Occupational Therapy and an active license in the practicing state, with ongoing continuing education to maintain

Qualifications

• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to inform treatment plans and goals.
• Interpret evaluation findings and communicate with residents, families, and the team.
• Establish individual therapy schedules with team.
• Conduct therapy per plan and document/bill services.
• Oversee COTA and ensure documentation and treatments meet standards.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
