Job summary

Evolve Therapy Services, LLC in Phenix City, AL, is hiring a dedicated Occupational Therapist for an in-house Skilled Nursing Facility on a PRN basis. The therapist will plan, organize, and direct OT services, ensuring compliance with federal, state, and local standards and delivering high-quality resident care.

Key duties include screening, evaluation, treatment planning, interdisciplinary communication, and supervision of COTA with accurate documentation and billing.

Qualifications

• Must have OT degree and active license in practicing state.
• Ongoing continuing education to maintain licensure.
• PRN role in an in-house Skilled Nursing Facility setting.

Responsibilities

• Screen residents to identify best intervention or treatment.
• Evaluate residents and develop short- and long-term goals.
• Communicate findings and plans to residents, families, and team.
• Establish resident therapy schedules with team members.
• Provide therapy and document and bill services accordingly.
• Oversee COTA and ensure compliant documentation and reimbursement.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient


• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals


• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members


• Establish individual resident therapy schedules in cooperation with other team members


• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services


• Perform all other duties as assigned and as required to effectively treat patients


• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy


• MUST have an Active Occupational Therapy license in practicing state(s)


• Ongoing continuing education and professional development to maintain current licensure and certification
