Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Phenix City, AL. This PRN role focuses on delivering high-quality OT services to residents and coordinating care with the interdisciplinary team.

Responsibilities include screening residents, evaluating needs, developing and monitoring treatment plans, scheduling therapy sessions, supervising COTA where appropriate, and ensuring accurate documentation and billing in compliance with standards.

Qualifications

• Bachelor degree or higher required in Occupational Therapy.
• Active OT license in practicing state(s) is mandatory.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop short- and long-term treatment plans.
• Communicate evaluation findings and recommendations to residents, families, and team members.
• Establish resident therapy schedules in coordination with the team.
• Conduct therapy per plan and document/bill OT services.
• Oversee COTA as needed and ensure documentation and reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
