Job summary

Evolve Therapy Services, LLC is seeking a compassionate Occupational Therapist for our partner facility in Selma, AL.

This in-house SNF position focuses on planning, directing, and delivering OT services, evaluating residents, writing treatment plans, and collaborating with an interdisciplinary team to ensure the highest standard of resident care. The role includes supervising COTA, ensuring documentation and billing accuracy, and maintaining licensure through ongoing continuing education.

Qualifications

• OT degree obtained from accredited program.
• MUST hold an active OT license in practicing state.
• Commitment to ongoing professional development.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed.

Job Type:

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:
