Job summary

Evolve Therapy Services seeks a dedicated Occupational Therapist to provide in-house OT services for a Skilled Nursing Facility in Phenix City, AL. This PRN position focuses on planning, directing, and delivering high-quality OT care with compliant documentation.

Responsibilities include screening, evaluating, developing treatment plans, coordinating with the interdisciplinary team, and supervising COTA as needed to ensure clinical excellence and proper reimbursement.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings to residents, families, and the inter-disciplinary team.
• Establish individual resident therapy schedules with the team.
• Perform therapy according to treatment plans and document/bill OT services.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
