Job summary

Evolve Therapy Services in Mobile, AL is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility on a PRN basis. You will plan, direct therapy services, and collaborate with a multidisciplinary team to deliver high-quality resident care.

The role requires an OT degree and an active license in the practicing state, with duties including screenings, evaluations, treatment planning, documentation, and supervision of COTA as needed.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• OT degree from accredited program.
• Commitment to ongoing professional development.

Responsibilities

• Screen residents to determine best intervention and treatment plan.
• Evaluate residents to inform goals and treatment plan progression.
• Communicate findings and plans to residents, families, and team.
• Schedule resident therapy in coordination with the team.
• Deliver therapy according to treatment plans and document services.
• Ensure proper billing and use of documentation.
• Supervise COTA and ensure compliant clinical practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
