Job summary

Evolve Therapy Services in Sutherland, NE seeks a licensed Occupational Therapist to provide in-house therapy at a Skilled Nursing Facility on a PRN basis. You will plan, implement and supervise OT services to residents, coordinating with a multidisciplinary team to ensure quality care.

The role requires a degree in Occupational Therapy, an active license in Nebraska, and ongoing continuing education. You will screen and evaluate residents, develop treatment plans with measurable goals, document

• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Screen residents to determine best intervention and treatment.
• Evaluate residents to inform treatment plans and goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team members.
• Establish resident therapy schedules in cooperation with the team.
• Deliver therapy per established plan and correctly document and bill services.
• Oversee COTA as needed and ensure documentation and treatments follow reimbursement practices.
• Perform other duties as assigned to effectively treat patients.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
