Job summary

Evolve Therapy Services is seeking an Occupational Therapist to support an in-house Skilled Nursing Facility in Sutherland, NE. This PRN role requires planning, directing, and delivering therapy to residents in compliance with regulations, while coordinating with a multidisciplinary team.

The therapist will assess and formulate treatment plans, establish goals, educate families, and supervise COTA as needed to ensure high-quality care and proper documentation and billing practices.

Qualifications

• OT degree required.
• Active OT license in the practicing state is mandatory.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to identify best intervention for each patient.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings to residents, families, and team.
• Set up therapy schedules with team members.
• Deliver therapy per plan and document/bill services.
• Oversee COTA and ensure proper documentation and reimbursement.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
