Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility partner located in Altoona, PA. This PRN role focuses on delivering high-quality therapy services and coordinating with the interdisciplinary team.

The Occupational Therapist will plan, develop, and direct OT services, ensure documentation and billing accuracy, and supervise COTA as needed to maintain clinical excellence and compliance with regulations.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to identify best intervention for each patient.
• Evaluate residents to inform treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the care team.
• Establish individual therapy schedules with input from the team.
• Deliver therapy per plan and ensure proper documentation and billing of services.
• Perform additional duties as needed to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement practices are followed.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

PRN

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
