Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility in Altoona, PA. This PRN position focuses on delivering high-quality OT services and collaborating with an interdisciplinary team to support resident care.

Responsibilities include resident screenings, creating and adjusting treatment plans, documenting outcomes, supervising COTA, and ensuring compliant reimbursement practices.

Qualifications

• Licensed OT in practicing state(s) with active license.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine the best intervention.
• Evaluate residents to inform and develop treatment plans with goals.
• Interpret and communicate evaluation findings and treatment plans to residents, families, and the team.
• Establish individual therapy schedules with other team members.
• Document and bill OT services accurately and timely.
• Perform all duties to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet clinical and reimbursement standards.

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
