Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Altoona, PA. This PRN position focuses on planning, organizing, developing, and directing OT services to ensure the highest quality resident care.

Responsibilities include screening and evaluating residents, developing treatment plans, communicating findings to families and the interdisciplinary team, scheduling therapy, documenting and billing OT services, and supervising COTA as needed.

Qualifications

• Must hold a degree in Occupational Therapy.
• Must have an Active Occupational Therapy license in the practicing state.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents; develop short- and long-term goals and plans.
• Communicate evaluation findings and recommendations to residents, families and team.
• Schedule resident therapy in coordination with team members.
• Deliver therapy per plan; document and bill OT services accurately.
• Perform other duties to effectively treat patients as assigned.
• Oversee COTA as needed and ensure documentation and reimbursement practices are followed.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
