Job summary

Evolve Therapy Services seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Clearfield, PA. The role requires planning, directing, and supervising OT services to ensure high-quality resident care.

Responsibilities include screenings, evaluating residents, developing treatment plans, scheduling therapy, and ensuring accurate documentation and billing. Licensure and ongoing CEUs are required.

Qualifications

• Must have Degree in Occupational Therapy.
• Must hold an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular resident screenings to determine best intervention for each patient.
• Evaluate residents to inform and develop treatment plans with goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families.
• Establish individual resident therapy schedules in collaboration with team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of OT services.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments follow reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full-Time, Part-Time, PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
