Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Clearfield, PA to deliver quality care and coordinate with a multidisciplinary team.

Must hold an OT degree and an active license in the practicing state; full-time, part-time, or PRN positions available with competitive pay and growth opportunities. Responsibilities include screening, evaluation, planning, and documentation.

Qualifications

• Must have OT degree and be licensed to practice in Pennsylvania.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screenings to determine best interventions.
• Evaluate residents to inform and develop treatment plans and goals.
• Communicate findings and plans to residents, families, and the team.
• Establish resident therapy schedules with the care team.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform duties to effectively treat patients as assigned.
• Oversee COTA and ensure documentation and reimbursement practices are followed.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full-Time, Part-Time, PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
