Job summary

LiveWell Homecare Agency, LLC is seeking an Occupational Therapist to provide intermittent skilled care in patients' homes in Rhode Island. You will develop treatment plans, enhance functional independence, and educate patients and families.

Collaboration with supervisors and adherence to care protocols are essential to ensure high-quality home health services. The ideal candidate is licensed in RI, graduate of an approved OT program, and has experience with home health or supervision-based

Qualifications

• Must be licensed as an OT in the current practice state.
• Graduate from an approved Occupational Therapy program.
• Preferably with at least one year of experience; Medicare home health experience is a plus.
• Ability to work under supervision and follow physicians’ treatment plans.

Responsibilities

• Provide professional, intermittent skilled care in home settings, focusing on improving patients’ functional independence and teaching family members as part of the care team.
• Develop treatment plans to enhance or compensate for loss of function.
• Educate patients, families, and healthcare team members on therapy techniques and progress.
• Collaborate with supervisors and adhere to care protocols.
