Job summary

LiveWell Homecare Agency, LLC in Champaign, IL is seeking an experienced Home Care Occupational Therapist to evaluate patients of all ages, develop and adjust treatment plans, and collaborate with the healthcare team to support independence in daily living at home.

You will supervise OTAs and aides, conduct comprehensive home assessments, and ensure accurate documentation and discharge planning, while educating families and maintaining regulatory compliance.

Qualifications

• Bachelor’s Degree in Occupational Therapy required.
• Licensed Occupational Therapist (IDFPR) required.
• BLS certification required.
• Valid Driver’s License and auto insurance required.
• Ability to work collaboratively in an interdisciplinary team.
• Strong assessment, documentation, and patient education skills.
• Knowledge of home care rules, regulations, and safety protocols.

Role Overview:

The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages, focusing on resuming or adjusting daily activities. They collaborate with the healthcare team, modify care plans as needed, and serve as a clinical resource and role model for staff and students. The therapist conducts comprehensive home assessments, educates patients and families, and ensures proper documentation of care, goals, and discharge planning. They evaluate patient function, recommend home safety modifications, and guide other health team members, including Occupational Therapy Assistants and caregivers. The role requires supervising OTA visits, completing assessments like OASIS, and maintaining compliance with home care regulations.
