TheraCare is seeking a licensed Occupational Therapist (OT) for a PRN home health opportunity in San Antonio, TX. You will provide in-home OT evaluations and one-on-one therapy to adults and seniors, focusing on mobility, safety, and independence in the home setting.

Flexibility to create your own schedule with autonomy and collaborative support from an interdisciplinary team. Strong clinical skills, effective communication, and a passion for home-based care are essential.

Qualifications

• Doctorate in Occupational Therapy from an accredited program.
• Active OT license in Texas or eligible Compact OT Licensure.
• Home Health experience (1 year required).
• Current CPR certification.
• Knowledge of HomeCare HomeBase (HCHB) preferred.

Responsibilities

• Perform home health OT evaluations and re-evaluations for adults and seniors.
• Develop and update Plans of Care (POC).
• Treat patients with orthopedic, neurological, cardiac, post-surgical conditions.
• Address ADLs, IADLs, balance, strength, and mobility.
• Apply manual therapy and modalities per physician orders.
• Document accurately in the EMR.
