TheraCare Home Health is seeking a licensed Physical Therapist (PT) to provide in-home, one-on-one physical therapy to adults and seniors in San Antonio and surrounding areas. This full-time role offers flexible scheduling and the autonomy to tailor care in the patient’s home environment.

You will evaluate, treat, and educate, coordinating with physicians and the care team to meet mobility and safety goals. CPR, Texas PT license, and home health experience are preferred.

Qualifications

• Licensed PT in Texas or eligible for compact licensure.
• Home health experience preferred; hospital experience may be considered.
• Strong communication and clinical judgment required.

Responsabilités

• Perform home health PT evaluations and re-evaluations including OASIS.
• Develop and update Plans of Care based on goals and physician orders.
• Provide orthopedic, neuro, cardiac and post-surgical PT interventions.
• Educate patients and caregivers on exercises, safety, and equipment.
• Monitor progress and adjust treatment plans; document in EMR at point of care.
• Supervise PTAs and home health aides; participate in care coordination.
