TheraCare is seeking a licensed Occupational Therapist (OT) for a Full-Time or PRN home health opportunity providing in-home therapy to adults and geriatric patients across Baytown, TX and surrounding areas.

You will deliver one-on-one in-home care, develop POCs, apply therapies, and support patients in mobility, safety, and independence in their homes. Flexible daytime schedules promote autonomy and professional growth.

Qualifications

• Doctorate in OT from an accredited program.
• Active OT license in Texas or eligible Compact license.
• Home Health experience (1 year required).
• Current CPR certification.
• Knowledge of home health regulations and Medicare guidelines preferred.

Responsibilities

• Perform home health occupational therapy evaluations and re-evaluations for adults and seniors.
• Develop and update individualized Plans of Care (POC).
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions.
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility.
• Apply manual therapy techniques and non-complex modalities (heat, cold, electrical stimulation, etc.).
• Implement falls prevention, home safety, and environmental modifications.
• Train patients and caregivers in adaptive equipment and DME.
• Fabricate splints and recommend assistive devices as appropriate.
• Complete accurate EMR documentation and communicate with physicians.
• Supervise COTAs and home health aides where applicable.
