TheraCare is seeking a licensed Occupational Therapist (OT) for a PRN home health opportunity in San Antonio, TX. You will provide one-on-one in-home therapy to adults and seniors, evaluating and treating a range of conditions in patients’ homes.

You will develop and update POCs, address ADLs/IADLs, and collaborate with physicians and an interdisciplinary team. Supervision of COTAs and aides is expected where applicable.

Qualifications

• OT licensure in Texas or eligible for Compact licensure.
• Home health experience (1 year required).
• CPR certification and clean driving record preferred.

Responsibilities

• Perform home health OT evaluations and re-evaluations.
• Develop and update Plans of Care (POCs).
• Treat orthopedic, neurological, cardiac, or post-surgical conditions.
• Address ADLs, IADLs, balance and functional mobility.
• Apply manual therapy and non-complex modalities as ordered.

Job Responsibilities

• Perform home health occupational therapy evaluations and re-evaluations for adult and geriatric patients
• Develop and update individualized Plans of Care (POC)
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility
• Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments
• Implement interventions for fall prevention, home safety, and environmental modifications
• Train patients and caregivers in adaptive equipment and durable medical equipment (DME)
• Fabricate splints and recommend assistive devices as appropriate
• Complete accurate and timely documentation in the EMR
• Communicate with physicians and interdisciplinary care teams
• Supervise COTAs and home health aides, as applicable
• Follow all home health regulations, infection control, and safety standards

Qualifications
