Job summary

Transition Home Healthcare in Houston, TX is seeking an Occupational Therapist (OT) to join our home health team. Shifts available PRN and FT.

Responsibilities include developing care plans, facilitating rehab, coordinating with the home health team, educating patients and families, and documenting progress.

Texas license required; home health experience preferred; valid driving record.

Qualifications

• Licensed OT in Texas.
• Home health experience preferred.
• Knowledge of home health regulations and compliance.
• Physically able to perform workload and drive to patient homes.
• Valid driving record.
• Experience with HCHB EMR is a plus.

• Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
• Develop and implement occupational therapy care plans.
• Coordinate care with home health care team.
• Evaluate therapy needs and monitor patient progress and outcomes.

• Prepare timely, complete clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers and team members.
