SummitWest Care in Mesa County, CO is seeking a compassionate Occupational Therapist (OT) to provide one-on-one home health care. This full-time role offers a flexible daytime schedule and mileage reimbursement, focusing on helping patients maintain independence at home.

You will perform evaluations, develop individual treatment plans, educate patients and caregivers, and collaborate with nurses and therapists.

Qualifications

• Colorado OT license in good standing.
• Graduate of an accredited Occupational Therapy program.
• Strong assessment, communication, and documentation skills.
• Ability to work independently in home health environment.

Responsibilities

• Perform occupational therapy evaluations, reassessments, recertifications, and discharges.
• Develop and implement individualized treatment plans focused on safety, function, and independence.
• Assess patients' ability to perform ADLs.
• Conduct home safety evaluations and recommend adaptive equipment or environmental modifications.
• Educate patients and caregivers on home programs, mobility strategies, and safety techniques.
• Monitor patient progress and adjust treatment plans as appropriate.
• Collaborate with nurses, therapists, caregivers, physicians, and family.
• Supervise and support COTAs when applicable.
• Complete accurate and timely clinical documentation.

SummitWest Care is a local nonprofit home health agency seeking a compassionate Occupational Therapist (OT) to provide one‑on‑one care throughout Mesa County. Home health offers a different therapy experience.

• Current Colorado Occupational Therapist (OT) license in good standing

• Excellent time‑management and organizational skills
• Valid driver's license, reliable transportation, and auto insurance

Home health experience is a bonus, but not required!
