Job summary

HealthFlex Home Health & Hospice in Fremont, CA seeks a Home Health Occupational Therapist to help patients regain independence in daily activities at home. You will assess needs, design individualized care plans, and implement therapeutic activities to improve function and safety in the patient’s living environment.

Requirements include a valid CA OT license, at least 1 year of OT experience, CPR certification, and a valid driver's license.

Qualifications

• Current and unrestricted CA OT License.
• At least 1 year of OT experience.
• Current AHA CPR certification.
• Valid driver’s license.

Responsibilities

• Help patients regain independence in daily activities in home care.

Position Summary

Home Health Occupational Therapists (OT) specialize in assisting patients to regain independence in daily activities. They assess individual needs, create customized plans, and implement therapies to improve functional abilities, adapt environments, and enhance quality of life, all within the comfort of patients' homes.
