Base pay: $120,000.00/yr – $130,000.00/yr

Full Time Occupational Therapist

Territory: Sacramento County – Elk Grove and surrounding communities.

What you’ll do as an Occupational Therapist (OT) Home Health

In partnership with the Case Manager, you will provide skilled services to evaluate and address the needs of individuals in their homes so they can live safely and productively. Services may include home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease management, dementia caregiver training, and more. You will serve as an educator, patient advocate, and member of the interdisciplinary care team.

To be a Success as an OT Home Health

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA driver’s license
• Knowledge of local community resources is required
• Knowledge of individual development and human behavior as it relates to illness/injury and the influence of culture on health care
• Knowledge of state and federal home‑care regulatory guidelines preferred
• Experience in home health care highly preferred
