Job summary

Concierge Home Care is seeking a Home Health Occupational Therapist (OT) to provide 1:1 therapy in patients’ homes across Lake County, FL. The role features a flexible schedule with 4 days per week, and a pay-per-visit model designed to maximize earnings.

Earn $70–$75 per visit plus Medicare differential pay, mileage reimbursement, and a data plan stipend. There is no weekend hospital shift, and clinicians can manage their own workload with strong clinical support.

Qualifications

• Active, unencumbered Florida OT license.
• Valid driver’s license, reliable transportation, and auto insurance.
• CPR certification.
• Ability to supervise COTAs.
• Home health experience preferred, but not required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs, functional mobility, and independence.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams.
• Supervise COTAs.
• Complete documentation using streamlined technology.
