Job summary

Tennessee Quality Care is seeking a compassionate Occupational Therapist (OT) to join our Home Health team in Columbia, TN and surrounding counties. This PRN role focuses on one-on-one patient care in home settings to enhance independence and daily function.

You will develop individualized treatment plans, monitor progress, and collaborate with physicians and the care team to ensure quality outcomes for patients living at home.

Qualifications

• Graduate of an accredited Occupational Therapy program approved by the AOTA.
• Current NBCOT certification and state licensure.
• At least two years of clinical OT experience preferred.
• Current CPR certification and reliable transportation.

Responsibilities

• Provide occupational therapy services per physician plan of care and state guidelines.
• Develop individualized plans and functional goals to improve independence.
• Communicate care plans with physicians and interdisciplinary team.
• Teach skills or retrain patients in activities of daily living.
• Fabricate splints and train use of adaptive equipment.
• Train patients in prosthetic and orthotic device use as appropriate.
• Maintain accurate clinical documentation and patient status updates.
• Notify changes in patient condition to care team.
• Collaborate with healthcare professionals for coordinated care.

What You’ll Do

• Provide occupational therapy services in accordance with the physician’s plan of care and state practice guidelines
• Develop individualized treatment plans and establish functional goals to improve patient independence
• Communicate plans of care with physicians and interdisciplinary team members
• Teach new skills or retrain patients in activities of daily living impacted by illness or injury
• Fabricate splints and instruct patients in the use of adaptive equipment to enhance function
• Train patients in the use of prosthetic and orthotic devices as appropriate
• Maintain accurate clinical documentation, including notes, reports, and updates to patient status
• Communicate changes in patient condition to the physician and care team
• Collaborate with other healthcare professionals to ensure coordinated and efficient patient care

Qualifications

• Graduate of an accredited Occupational Therapy program approved by the American Occupational Therapy Association (AOTA)
• Current registration through the National Board for Certification in Occupational Therapy (NBCOT) and licensed in the state of practice
• At least two (2) years of clinical experience as an Occupational Therapist (preferred)
• Current CPR certification
• Reliable transportation, valid driver’s license, and current automobile insurance
