Job summary

AdviniaCare is seeking a compassionate Occupational Therapist to join our rehab team in the Wellesley/Newton area. You will build meaningful relationships with residents, families, and an interdisciplinary team while helping residents regain independence in daily activities.

Qualifications

• Active OT license in good standing.
• Patient-centered approach with emphasis on independence.
• Collaborative care within interdisciplinary teams.

Responsibilities

• Assess resident needs and develop individualized treatment plans.
• Implement and adjust plans addressing physical, cognitive, and emotional goals.
• Collaborate with the team to ensure coordinated, relationship-based care.
• Educate residents and families on exercises and techniques.
• Monitor progress and document outcomes.
• Maintain accurate patient records.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with the interdisciplinary team to ensure coordinated, relationship-based care
• Educate residents and families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate, up-to-date patient care records
