Job summary

AdviniaCare in Massachusetts is seeking a compassionate Occupational Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans that promote independence, safety, and quality of life for residents.

You will assess resident needs, modify plans as needed, collaborate with the care team, educate residents and families on exercises, monitor progress, and maintain accurate records to ensure high-quality care.

Qualifications

• Licensed Occupational Therapist with active license.
• Experience in an interdisciplinary rehab setting preferred.
• Ability to design individualized treatment plans to promote independence, safety and quality of life.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
