Job summary

Pleasant Hill Post Acute is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team in Pleasant Hill, CA. The role offers a collaborative, patient-centered environment with an interdisciplinary team and opportunities to mentor new grads.

Qualifications

• Current California Occupational Therapist license in good standing.
• Strong clinical reasoning, communication, and organizational skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and goals.
• Provide skilled occupational therapy services to improve independence with activities of daily living.
• Help patients maximize functional abilities and improve quality of life.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, physical therapists, speech therapists, and other team members.
• Communicate effectively with patients, families, and caregivers regarding treatment goals and progress.
• Complete accurate, timely, and compliant clinical documentation.
• Provide compassionate, patient-centered care while maintaining professional standards.
• Participate in interdisciplinary care planning and discharge planning as appropriate.

What You'll Do

Qualifications
