Job summary

Pleasant Hill Post Acute is seeking a compassionate Occupational Therapist to join our rehabilitation team in Pleasant Hill, CA. We offer Full-Time, Part-Time, and PRN opportunities with pay ranging from $60–$66+ per hour based on experience.

Responsibilities include evaluating patients, designing individualized treatment plans, and delivering skilled OT services to improve independence in daily activities and overall quality of life.

Qualifications

• Current California Occupational Therapist (OT) license in good standing.
• Strong clinical, communication, and organizational skills.
• Ability to work effectively as part of an interdisciplinary team.
• Skilled nursing or post-acute rehabilitation experience preferred.
• New graduates are welcome to apply.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled occupational therapy services to help patients regain independence in ADLs.
• Help patients maximize functional abilities and quality of life.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, PTs, SLPs and other team members.
• Communicate treatment goals and progress with patients, families and caregivers.
• Complete accurate, timely, and compliant clinical documentation.
• Provide compassionate, patient-centered care while maintaining professional standards.
