Job Duties and Responsibilities
- Perform Occupational Therapy evaluations, complete care plans, establish goals, and communicate frequently with Clinical Occupational Therapy Assistants (COTAs) who assist in implementing these care plans and goals.
- Effectively communicate with the Director of Rehabilitation (DOR), therapy staff, and nursing staff regarding patient progress, barriers, and treatment plans.
- Verify patient information and appropriately chart, including therapy notes, treatment times, and labor hours.
Qualifications
- Must possess a current and valid fingerprint clearance card (FPCC).
- Must possess a current and valid Occupational Therapy license.
- Must possess a current and valid CPR certification.
- Must pass a pre‑employment background check and drug test.
- 1 year’s experience in a skilled nursing facility is preferred.

- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to tactfully work with personnel, residents, family members, visitors, government agencies and the general public.
- Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.
- Must be able to relate information concerning a resident’s condition.
- Must be knowledgeable of HIPAA compliance and requirements, and have the ability to maintain highly confidential information.
- Must not pose a direct threat to the health and safety of other individuals in the workplace.
