Job summary

Hearthstone Nursing & Rehabilitation Center in Medford, OR is seeking an Occupational Therapist (OT) to evaluate patients, design treatment plans, and coordinate care with the interdisciplinary team.

This role offers full-time, part-time and PRN/on-call options with hourly pay between $48 and $65, plus benefits including medical/dental/vision, 401(k), PTO, tuition reimbursement and licensure support.

Qualifications

• OT licensure in the appropriate state as required.
• Bachelor’s degree preferred; strong communication skills.
• Ability to create and implement individualized treatment plans.
• Accurate documentation and timely billing compliance.
• Willingness to meet productivity benchmarks.

Responsibilities

• Screens, evaluates, and develops treatment programs for patients as needed.
• Designs effective OT treatment plans and goals to meet patient needs.
• Collaborates with interdisciplinary team to ensure optimal outcomes.
• Maintains productivity benchmarks and completes documentation on time.

Salary

• $48-$65 per hour, depending on licensure and experience

Schedule

• Full-time, Part-time and PRN/On-Call shifts are available!

Skills & Abilities

What We’re Looking For

• Occupational Therapist licensure as outlined by the state Board of Occupational Therapy
