Job summary

LiveWell Homecare Agency, LLC is seeking a licensed occupational therapist to provide direct OT services in a post-acute care setting in Mesa, AZ. You will develop and adjust treatment plans based on physician referrals and collaborate with interdisciplinary teams to meet patient goals.

Maintain accurate progress notes, supervise OTAs and support staff, ensure safety and regulatory compliance, and participate in community health initiatives while delivering compassionate, client-centered care.

Qualifications

• NBCOT certification (renewal optional).
• Graduate of an accredited occupational therapy program (BSOT or MSOT).
• Active state licensure and CPR certification.
• Preferred: 1+ year experience; new grads welcome.
• Proficient in Microsoft Word, Excel, and email.
• Strong communication, decision-making, and body mechanics skills.
• Knowledge of laws, regulations, and safety procedures in post-acute care.

Responsibilities
