Job summary

Proforma Health is seeking an Occupational Therapist to provide direct rehabilitation in skilled nursing facilities, focusing on long-term care and short-term rehabilitation for patients with orthopedic, neurological, and cardiopulmonary conditions.

You will collaborate with an interdisciplinary team to develop, assess, and modify treatment plans to improve functional abilities and quality of life, while supervising OT staff and maintaining thorough documentation.

Qualifications

• Graduate of an accredited occupational therapy program (BSOT or MSOT).
• Current state licensure and NBCOT certification (renewal optional).
• CPR certification required; maintained during employment.

Responsibilities

• Implement and monitor individualized occupational therapy programs based on physician referrals.
• Establish, evaluate, and adjust patient goals to ensure measurable progress.
• Supervise and evaluate care provided by OT assistants, aides, and students.
• Maintain accurate, timely documentation and utilize therapy software effectively.
• Communicate with patients, families, and support staff to coordinate care and meet patient needs.
• Ensure adherence to safety, sanitation, and compliance standards, maintaining confidentiality.
