Job summary

Interim HealthCare in Colorado seeks a part-time Home Health Occupational Therapist to deliver client-centered OT in patients' homes. You will work with a home health team, assess function, develop goals, and document progress, while promoting safety and independence.

Experience with OASIS is preferred but training is available; strong communication and compassion are essential to support patients and families across diverse home care settings.

Qualifications

  • Graduate of an accredited Occupational Therapy program with active CO OT license.
  • Minimum 1 year in occupational therapy, preferably home health.
  • CPR/BLS certification required; knowledge of home health regulations.

Responsibilities

  • Provide occupational therapy to patients at home who cannot perform daily tasks.
  • Collaborate with a home health team (RN, LPN, CNA, HHA, PT, SLP) on plans of care.
  • Assess patients, set goals, document progress, and adapt therapy as needed.
  • Assist with exercises to improve fine motor skills and coordination.
  • Suggest adaptive equipment and assess fall risks to improve safety.
  • Educate patients and families on care plans, exercises, and self-care.

Home Health Occupational Therapist (OT)

We are seeking an energetic, self-motivated, hardworking, and dependable Occupational Therapist (OT) to provide exceptional care to clients in the comfort of their own homes. The ideal candidate is organized, adaptable, and able to multitask effectively.

The successful candidate will be comfortable working independently, demonstrate strong clinical and communication skills, and be committed to delivering high-quality, compassionate patient care.

Experience with OASIS documentation is preferred; however, we are happy to provide training for candidates who are willing to learn.

As a Home Health Occupational Therapist (OT), here’s a big-picture view of what you’ll do:
  • Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury.
  • Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and SLP, focused on the patient’s plan of care and personal goals.
  • Assess patient, observe deficits, establish therapy goals and document progress.
  • Assist patient with exercises to improve fine motor skills and coordination.
  • Suggest adaptive equipment such as grab bars and shower chairs to offer added support.
  • Assess fall risks and introduce strategies to improve home safety.
  • Educate patient and family on plan of care, exercises, goals and self-care.
A few must-haves for Home Health Occupational Therapists:
  • Graduate of an accredited Occupational Therapy Program and active OT license in the State of Colorado.
  • Minimum of 1 year of occupational therapy experience, preferably in home healthcare.
  • CPR/BLS certification (must be a demonstration course).
  • Knowledge of state and federal home health regulations.
  • Good clinical judgment, strong interpersonal skills, resourceful and compassionate.
