Job summary

Interim HealthCare is hiring a Home Health Occupational Therapist to deliver personalized in-home therapy in the Fort Collins area. You will assess, treat, and educate patients to regain independence and improve daily activities within a home setting.

The role emphasizes a 1:1 patient ratio, with per-visit pay ($75–$150) and travel reimbursement. Candidates must hold a Colorado OT license, have CPR/BLS, and a minimum of one year in home health or similar settings.

Qualifications

  • Graduate of an accredited Occupational Therapy Program and active OT license in the State of Colorado.
  • Minimum of 1 year of occupational therapy experience, preferably in home healthcare.
  • CPR/BLS certification (must be a demonstration course).
  • Knowledge of state and federal home health regulations.
  • Good clinical judgment, strong interpersonal skills, resourceful and compassionate.

Responsibilities

  • Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
  • Work as part of a home health team focused on the patient’s plan of care and personal goals.
  • Assess patient, observe deficits, establish therapy goals and document progress.
  • Assist patient with exercises to improve fine motor skills and coordination.
  • Suggest adaptive equipment to enhance daily living activities.
  • Assess fall risks and implement home safety strategies.
  • Educate patient and family on plan of care, exercises, goals and self-care.
