Job summary

Qualifications

Responsibilities

• Evaluate residents and develop individualized OT plans of care.
• Provide skilled OT services to improve mobility, strength, coordination, and independence in ADLs.
• Establish short-term and long-term treatment goals and monitor progress.
• Complete timely evaluations, progress notes, treatment documentation, and discharge summaries.
• Educate residents, families, caregivers, and staff on therapeutic techniques and recommendations.
• Collaborate with physicians, nursing, rehab staff, and the interdisciplinary care team.
• Recommend and train residents in the use of adaptive equipment.
• Ensure services and documentation meet facility standards and applicable state and federal requirements.

Full-Time Occupational Therapist (OT) – Skilled Nursing Facility

Lilac Manor | Rochester, NY

Position: Full-Time

Compensation: $45–$50 per hour

Lilac Manor is seeking a dedicated Full-Time Occupational Therapist (OT) to join our rehabilitation team. The Occupational Therapist will provide individualized, patient-centered care to help residents improve functional independence and achieve their highest level of ability in a supportive skilled nursing environment.

Responsibilities

• Evaluate residents and develop individualized occupational therapy plans of care based on clinical and functional needs.
• Provide skilled occupational therapy services to improve mobility, strength, coordination, and independence with activities of daily living.
• Establish appropriate short-term and long-term treatment goals and monitor resident progress.
• Complete timely and accurate evaluations, progress notes, treatment documentation, and discharge summaries.
• Educate residents, families, caregivers, and facility staff on therapeutic techniques and recommendations.
• Collaborate with physicians, nursing, rehabilitation staff, and other members of the interdisciplinary care team.
• Recommend and train residents in the use of adaptive equipment when appropriate.
• Ensure services and documentation meet facility standards and applicable state and federal requirements.

Qualifications
