Job summary

Aura Health is seeking a Home Health Occupational Therapist to deliver personalized OT services in patients' homes. You will assess needs, set therapy goals, document progress, and collaborate with RNs, LPNs, and other professionals to tailor care plans.

You will educate patients and families on routines and safety, recommend adaptive equipment, address fall risks, and support independent living while maintaining compassionate, patient-centered care.

Qualifications

• Graduation from an accredited OT program with active CA OT license.
• At least 1 year of OT experience, preferably in home health.
• CPR certification and knowledge of home health regulations.

Responsibilities

Home Health Occupational Therapist (OT)

In this role, you will provide personalized occupational therapy to patients in their homes, helping them regain independence and perform daily activities. You will collaborate with a healthcare team including RNs, LPNs, and other specialists to develop and implement care plans tailored to individual goals. Your duties include assessing patient needs, establishing therapy objectives, documenting progress, assisting with exercises, recommending adaptive equipment, assessing fall risks, and educating patients and families on care plans and safety strategies.

Qualifications Include

• Graduation from an accredited Occupational Therapy Program and active CA OT license
