Job summary

Berlin Health & Rehabilitation Center in Barre, Vermont, is seeking a dedicated full-time Occupational Therapist (OT) to join our in-house rehabilitation team. Pay ranges from $42–$55 per hour and new graduates are welcome to apply.

We provide person-centered care to improve residents’ independence, safety, and quality of life. The interdisciplinary team collaborates to deliver high-quality, compassionate care in a supportive environment.

Qualifications

• Active and valid Vermont OT license.
• Graduate of an accredited Occupational Therapy program.
• Experience in SNF or long-term care preferred.
• Familiarity with Net Health or similar EHR.

Responsibilities

• Evaluate residents and patients to determine OT needs and goals.
• Develop individualized treatment plans based on evaluations and orders.
• Provide OT interventions for ADLs, mobility, strength, cognition, balance.
• Create personalized rehab programs to improve independence and safety.
• Supervise OT Assistants per regulations and standards.
• Document evaluations, treatment notes, progress, and discharge planning.
• Collaborate with physicians and care team to coordinate care.
• Communicate changes in patient condition to the team.

Occupational Therapist (OT)

Berlin Health & Rehabilitation Center is seeking a dedicated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Barre, Vermont.

Pay: $42–$55 per hour

We provide person-centered care focused on helping residents and patients improve functional independence, safety, and quality of life. Our interdisciplinary team works collaboratively to provide high-quality, compassionate care.

Occupational Therapist Responsibilities

• Evaluate residents and patients to determine their occupational therapy needs, functional limitations, and rehabilitation goals.

• Develop individualized treatment plans based on evaluation findings, physician orders, patient needs, and established standards of care.

• Provide skilled occupational therapy interventions to address activities of daily living (ADLs), functional mobility, strength, coordination, cognition, balance, and other areas of occupational performance.

• Create personalized rehabilitation programs designed to improve functional independence, safety, and quality of life.

• Administer medically necessary occupational therapy treatments in accordance with the established plan of care.

• Monitor patient progress and response to treatment and modify interventions as clinically appropriate.

• Establish and update individualized goals based on patient progress and clinical findings.

• Provide education, training, and support to residents, patients, families, and caregivers regarding treatment plans, adaptive techniques, safety, and strategies to promote independence.

• Collaborate with physicians, nursing staff, therapy team members, and other members of the interdisciplinary team to coordinate quality patient care.

• Supervise and provide guidance to Occupational Therapy Assistants in accordance with applicable regulations and professional standards.

• Communicate changes in patient condition, progress, and treatment needs to the appropriate members of the healthcare team.

• Complete evaluations, treatment notes, progress reports, and other required documentation accurately and timely.

• Maintain accurate records related to treatment, goals, progress, and discharge planning.

• Comply with applicable federal and state regulations, professional standards, and facility policies.

• Participate in required meetings and other duties as assigned by the Director of Rehabilitation.

Occupational Therapist Qualifications

• Active and valid Occupational Therapy license in Vermont.

• Graduate of an accredited Occupational Therapy program.

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Current knowledge of occupational therapy treatment practices, techniques, and interventions.

• Familiarity with Net Health or similar electronic documentation systems preferred.

• Strong clinical, communication, organization, and interpersonal skills.

• Ability to work effectively with residents and patients with a variety of needs and abilities.

• Strong understanding of functional assessment, rehabilitation, and strategies to promote independence.

• Ability to collaborate effectively with an interdisciplinary healthcare team.

• Commitment to providing compassionate, patient-centered care.

• Must be in good standing with all applicable licensing boards and regulatory agencies.
