Job summary

Valley Hill Health and Rehab is seeking an Occupational Therapist to help residents regain function and achieve therapy goals in a skilled nursing facility. You will educate patients in independent exercise and activity modification to support discharge planning.

We require a Bachelor’s or Master’s in Occupational Therapy and an active OT license, plus basic computer and EHR skills for documentation and care coordination.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy from an accredited program.
• Current and valid OT license for the state of employment.
• Proficiency with electronic health records and basic computer skills.

Responsibilities

• Help patients regain physical function in SNF settings and support therapy goals.
• Educate patients and families on exercises and activity modification for discharge.
• Collaborate with physicians and staff to optimize care plans.

Position Summary

The primary function of your job is to return patient to optimal physical function and facilitate the achievement of patients, physicians, and therapy goals within an SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification.

Education / Qualifications

• Employee must possess a Bachelor’s Degree or Master’s Degree in Occupational Therapy from an accredited college or university.
• Must possess a current and valid license granted from state of employment.
• Must possess basic computer skills and the ability to navigate through various software programs, including service delivery software and the electronic health record software.
