Job summary

Jobtailor is seeking an Occupational Therapist to perform comprehensive evaluations and develop personalized care plans for home health patients in Michigan. The role emphasizes assessment of ADLs, cognition, strength, balance, safety, and home environment, with ongoing plan adjustment as needed.

You will educate patients and caregivers, train on adaptive equipment use, and collaborate with the care team to achieve optimal outcomes. Michigan licensure is required. Remote work is not indicated.

Qualifications

• Degree from an accredited OT or OTA program.
• Current Michigan OT or OTA license in good standing.
• COTA certification, as applicable.
• Home health experience preferred.
• Strong clinical judgment and problem-solving skills.
• Excellent communication and patient education skills.
• Ability to work independently while remaining connected to the care team.
• Reliable transportation, valid driver’s license, and current auto insurance.
• Familiarity with Medicare/CMS documentation preferred.

Responsibilities

• Complete comprehensive evaluations and identify functional limitations (OTs).
• Develop individualized plans of care and measurable treatment goals (OTs).
• Assess ADLs, cognition, strength, coordination, safety, and home environment (OTs).
• Reassess patients and modify treatment plans as clinically appropriate (OTs).
• Recommend appropriate adaptive equipment and home modifications (OTs).
• Provide skilled occupational therapy in accordance with physician orders and CMS Conditions of Participation.
• Provide ADL/IADL retraining, therapeutic exercise, functional mobility, balance, coordination, and upper-extremity interventions.
• Educate patients and caregivers on fall prevention, energy conservation, adaptive techniques, and home safety.
• Train patients in the safe use of adaptive equipment and durable medical equipment.
• Maintain timely and accurate documentation.
• Collaborate with physicians, caregivers, and the interdisciplinary team to support successful patient outcomes.
