Job summary

Home Care Wellness is seeking an Occupational Therapist to provide skilled home health services, evaluating patients in their homes and creating individualized care plans to promote independence in ADLs and safety.

The OT will conduct initial evaluations, implement treatment plans, and perform home safety assessments, with a focus on improving functional ability and patient independence. Michigan licensure, CPR, and a valid driver's license are required.

Qualifications

• Must hold a licensed OT credential in Michigan.
• Minimum 1 year of clinical experience in home health preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive initial evaluations to assess patient functional status (ADLs, mobility, cognition, home safety).
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Provide skilled OT interventions to improve independence in ADLs and IADLs.
• Conduct home safety assessments and recommend adaptive equipment and environmental modifications.
• Educate patients and caregivers on disease management and use of assistive devices.
• Complete OASIS assessments, progress notes, and discharge summaries timely and accurately.
• Communicate with physicians and interdisciplinary team regarding status and plan of care.
• Participate in care coordination and case conferences and ensure regulatory compliance.
• Maintain productivity standards while delivering high-quality patient care.

Job description

The Occupational Therapist (OT) provides skilled therapy services to patients in their homes, focusing on improving independence in daily living activities, safety, and overall functional ability. The OT evaluates patient needs, develops individualized care plans, and collaborates with the interdisciplinary team to promote optimal outcomes.

Perform comprehensive initial evaluations to assess patient functional status, including ADLs, mobility, strength, cognition, and home safety

Develop and implement individualized treatment plans based on physician orders and patient goals

Provide skilled occupational therapy interventions to improve independence in activities of daily living (ADLs) and instrumental ADLs

Conduct home safety assessments and recommend adaptive equipment and environmental modifications

Educate patients and caregivers on disease management, safety, and use of assistive devices

Complete all required documentation, including OASIS assessments, progress notes, and discharge summaries, in a timely and accurate manner

Communicate regularly with physicians, nurses, and other team members regarding patient status and plan of care

Participate in care coordination and case conferences

Ensure compliance with all Medicare, state, and agency regulations

Maintain productivity standards while delivering high-quality patient care

• Graduate of an accredited Occupational Therapy program
• Current Occupational Therapist license in the State of Michigan
• Minimum 1 year of clinical experience (home health experience preferred)
• Valid driver’s license, reliable transportation, and auto insurance
• Current CPR certification
• Strong clinical, communication, and documentation skills
• Experience with OASIS documentation preferred

Compensation

$55 - $75 hourly
