Job summary

Jobtailor is seeking an experienced Occupational Therapist to join our Iowa care team. You will evaluate patients, develop and implement care plans, and re-evaluate needs across visits to ensure quality and proper discharge planning.

duties include coordinating with physicians, supervising staff, and documenting goals in therapy notes. The role requires travel about 50% and lifting up to 50 pounds in a fast-paced clinical setting.

Qualifications

  • Graduate of an accredited Occupational Therapy program.
  • Current, unrestricted OT license valid for the state of work.
  • Minimum one year of experience in a clinical care setting.
  • Must be able and willing to travel 50% of the time.
  • Ability to frequently lift, push, pull, and move up to 50 pounds.
  • Able to work in a physically demanding, high-stress environment.

Responsibilities

  • Evaluate occupational therapy patients to determine rehabilitation needs and potential for achievement.
  • Develop plans of care based on patient evaluations and implement Plans of Care.
  • Perform initial therapy evaluation visits and re-evaluate patients during each visit.
  • Complete follow-up assessments according to OASIS and PPS guidelines.
  • Communicate significant findings to supervisors, physicians, and care team members.
  • Initiate patient safety plans using patient, family, and community resources.
  • Verify Plans of Care before each visit and provide care per physician orders.
  • Write physician orders for additional visits and changes to Plans of Care.
  • Document patient care goals and therapy services in notes.
  • Provide occupational therapy services per standards of practice and physician certification.
  • Instruct patients/families in ADLs using adaptive equipment and fine motor skills.
  • Assist patients in securing necessary equipment, including splints.
  • Instruct patients in home therapy programs and provide modifications.
  • Counsel and instruct patients and families regarding therapy needs.
  • Supervise COTAs, HHAs, and clinical staff; report to leadership.

Job description

  • Evaluate occupational therapy patients to determine rehabilitation needs and potential for achievement
  • Develop plans of care based on patient evaluations
  • Implement and develop Plans of Care to ensure quality, continuity of care, and proper discharge planning
  • Perform initial therapy evaluation visits and re-evaluate patients’ therapy needs during each visit
  • Complete follow-up assessments according to OASIS and PPS regulatory guidelines
  • Communicate significant findings, problems, and changes in patient condition or environment to supervisors, physicians, and care team members
  • Initiate patient safety plans using patient, family, and community resources
  • Verify Plans of Care before each visit and provide care according to physician orders, assessment data, and established standards and guidelines
  • Write physician orders for additional visits and changes to Plans of Care according to agency policy
  • Document patient care goals and therapy services in therapy notes
  • Provide occupational therapy services in accordance with accepted standards of practice and physician certification
  • Instruct patients and families in activities of daily living using adaptive equipment, sensory integration, and fine motor coordination skills
  • Assist patients and caregivers in securing necessary equipment, including custom-made splints
  • Instruct patients and caregivers in therapy home programs and provide modifications and clarification
  • Counsel and instruct patients and families regarding therapy and related needs
  • Supervise and provide clinical direction to COTAs, HHAs, and clinical staff
  • Report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee
Requirements
  • Graduate of an accredited Occupational Therapy program
  • Current, unrestricted OT license valid for the state of work
  • Minimum one (1) year of experience in a clinical care setting
  • Able and willing to travel within branch/office coverage area
  • Dependable vehicle
  • Valid driver’s license
  • Current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Able to work in a physically demanding, high-stress environment
  • Full range of body motion, including handling, lifting, and transferring patients
  • Potentially work irregular hours including call hours if applicable
