Job summary

Sweetwater Care and Rehab is seeking an Occupational Therapist to deliver rehabilitative therapy to residents, following regulatory and clinical standards. You will screen patients, develop plans of care, document progress, and collaborate with an IDT to support recovery and return to prior functioning.

The role reports to the Director of Rehabilitation Services and requires state licensure; NBCOT/AOTA alignment and compliance with professional practice acts are emphasized.

Qualifications

• Active state licensure required.
• NBCOT/AOTA alignment with state practice acts.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Provide rehabilitative, skilled and medically necessary therapy per standards.
• Document outcomes and progress in the medical record.
• Supervise assistants and rehab aids as needed.
• Participate in required trainings and meetings.
